Comment on insurance information for non-invasive prenatal testing (NIPT) in Germany [Kommentar zur Versicherteninformation „Bluttest auf Trisomien.

Abstract
The information for persons with health insurance in Germany: ‘Blood test for trisomies. The non-invasive prenatal test (NIPT) for trisomies 13, 18 and 21', issued by the 'Federal Joint Committee' (Gemeinsamer Bundesausschuss) in 2021, is intended to provide expert help and guidance to pregnant women. A review of the text, which is presented here, revealed that 37 points were unclear and/or would require correction according to the current literature. These issues include the origin of 'free fetal DNA', which is indeed derived from the placenta; the relation to the Genetic Diagnostics Act (Gendiagnostikgesetz); and the fact that a NIPT for trisomy 18 also tests for tetrasomy 18p syndrome. Furthermore, the status of NIPT as a screening test rather than a diagnostic test and the causes of false-positive results are not clearly presented. As the Federal Joint Committee's information for persons with health insurance in Germany "must be used by doctors in consultations on NIPT for trisomies 13, 18 and 21", a rapid revision seems necessary. As this information on the 'Federal Joint Committee' is published in German and is relevant to German doctors and public, this article has been exceptionally published in German language. A German abstract is provided below.

Key words: Genetic Diagnostics Act; screening test; non-invasive prenatal testing (NIPT); cell-free fetal DNA (cffDNA); tetrasomy 18p syndrome
